OSHA:

The OSHA respiratory physical includes a basic health evaluation with review of past and current medical history. There is also an extensive questionnaire that is to be completed proir to the exam.   The first section (A) is comprised of a 2 part basic questionnaire and (B) deals with "on the job" exposure and work environment.
